Our Services

We are an ADW (Aged and Disabled Waiver), state certified, CASE MANAGEMENT AGENCY providing services to ADW participants in the Upshur, Lewis, Barbour, Randolph and Webster Counties. Our case managers have experience with ADW & Medicaid paperwork and extensive knowledge of the same.

Medicaid is a State and Federally funded Program for eligible individuals and families. You apply for Medicaid through your local DHHR office.

Aged and Disabled Waiver (ADW) is a program within the Medicaid program. It was designed to keep individuals that are at a nursing home level of care at home, as opposed to a nursing home. You apply for ADW through your family physician.

What we offer

Case Management Services and Case Managers

Case Management includes the coordination of services that are individually planned and arranged for people whose needs may be lifelong. The practice of case management helps to avoid duplication and provision of unnecessary services, and to ensure a balance of services.

We provide Case Management Services for the elderly and disabled in Upshur, Lewis, Barbour, Webster and Randolph Counties. Our Case Managers are experienced professionals who assess and identify you or your loved one’s needs, then help to identify a plan for care. Our Case Managers will also be advocates for you or your loved one in your time of need.

Case Managers can be licensed in West Virginia as a social worker, licensed professional counselor, RN, or they may also possess a four-year degree in an approved Human Services field.

Case Managers meet you in person, in your home, and assess you, upon your request. You choose your case manager on the Case Management Agency Selection Form when you sign up for the ADW program.

Upon meeting you, they assess you and your needs to maintain an independent lifestyle at your home, instead of a nursing home.

Your reasonable goals in accordance with the ADW program, then become your case manager’s goals. The case manager then networks within local community resources and organizations to maintain your goal of living independently at your home.
